Buttons designated as "*Free destination form" include the option to use our free form submission hosted by Packed Planet. To opt in, simply leave the default Packed Planet URL in place. Form submissions will send an email alert to the email destination provided. You may also replace the default URL with your own custom destination, or leave it blank to opt out.

If your Virtual Kiosk is unused for an extended period of time, Packed Planet may use its discretion to revoke MyQiosk from Client with 30 days' notice. If you need to update or make changes to your existing MyQiosk page, please do so using this form or contacting your account rep.
